This policy explains how Horizon Health Collective LLC (“we”) handles information collected through this website.
What we collect
- Information you send us through our contact, referral and careers forms or by email — such as your name, email, phone number and the details you choose to share.
- Portal accounts — name, email and the documents you or our staff add to your portal.
- Technical data — standard server logs (IP address, browser type, pages visited) kept for security.
What we don’t want through the website
Please don’t send medical records or clinical details through our website forms or email. Once we begin working together, we’ll set up a secure channel for that information.
How we use it
To respond to you, arrange services, consider job applications and keep the site secure. We don’t sell personal information or use it for advertising.
Sharing
We share information only with service providers who help us run the website and email, when you ask us to (for example, with a case manager), or when the law requires it.
Retention and security
Form messages are delivered by email and aren’t stored in the website database. We keep information only as long as needed for the purpose it was collected, and we protect it in line with Massachusetts data-security law (201 CMR 17.00).

Clients of our home care services receive a separate Notice of Privacy Practices as part of their admission packet.
