Care Coordination

One point of contact who keeps the plan current and everyone informed.

  • Assessment and service plan — a written plan based on an in-home assessment, reviewed with you and the person receiving care.
  • Supervisory visits — regular visits to check the plan is working and the caregiver is the right fit.
  • Change-in-condition reporting — if a caregiver notices something new, it’s documented and passed on the same day.
  • Working with others — with your permission, we communicate with physicians, case managers and discharge planners.
  • Family updates — regular check-ins so relatives near or far know how things are going.
